There's basically two routes:

1) Learn a programming language. This is not something you can do in a day, or even in many days. It takes a long time and a lot of work. So, if you've never done any programming before, you'll probably want to try option 2 ...

2) Use a game creation tool. Probably <a href="http://www.yoyogames.com/gamemaker/">GameMaker</a> is what you're looking for, or you can browse the <a href="http://www.ambrosine.com/resource.html">huge list of game creation systems</a>.

You can also create whole new DD levels.
For that you need of course a level editor.
You can find it @ Romero's home site:
http://www.rome.ro/games_ddici.htm

Download "DANGEROUS DAVE IN COPYRIGHT INFRINGEMENT" , run it in dosbox, (unzip it first *LOL*).
TED (the level editor) is included.
Works pretty good.

off topic:

Wow, so that's why it's rome.ro... I've stumbled upon it once, and wondered why in the world it had nothing to do with Rome

I feel smarter now!

Well, he posted about our forum in his forum. And, that's exactly the meaning of "visited." If I meant "he posted here" I would've said that instead.
